Get your personal records organized. This program will help by prompting you for a wide range of details, including your medical history, medications, physicians, etc. The resulting output provides the information you will need to complete medical information and medical history forms at your doctor's office. It will also help your doctor better understand your medical history and current medical conditions, giving them the information they need to properly treat you.
Once the data is entered, it is automatically transferred into five output options for printing. There are one and two page formats, depending on the amount of information supplied. The one page output is already formatted and should accommodate all of the information for most individuals. For those familiar with Microsoft Excel, the look of some of the data can be personalized in a user format output option. If, however, you need more space for your information, there is a two page option with expanded room for prescriptions, physicians, and medical history/information. As with the one page option, there is a two page user format output option.
Lastly there is a wallet option that provides two identical reduced sized outputs that can be cut and folded into a business card size to carry with you for instantaneous personal reference.
Your medical situation will dictate which outputs are best suited for you. The data entry is the same regardless of the print options chosen.
All the information can be corrected or updated at any time and new medical sheets printed.
